Payment Options
At The Center for Cosmetic Dentistry, we want you to be fully informed in order to make a decision about your dental investment. The following are the payment options available to you for your dental treatment:
- 5% discount for payment in full at the start of treatment with cash or check.
(Please note: check/debit cards are considered credit cards, not cash or check, discount is not applicable with any other discounts or coupons.)
- Master Card/Visa, American Express, Discover (5% discount is not applicable.)
- Dental Fee Plan
Flexible payment plan that offers monthly payments with a range of payment terms and interest rates from 5.9% to 12.9%. These rates are fixed and lower than most credit cards. Requires no down payment. Fast and convenient. No application forms.
Simply go to www.dentalfeeplan.com or call 1-888-337-4171 and a representative will have a decision for you in 15 minutes ($1,000.00 minimum.)
(Please note: Other credit programs may be available through DFP for applicants not approved through the primary program.)
- Sleep Dentistry appointment fees are due in full at the time medication is dispensed. 48 hours notice is required for cancellations.
Insurance will be submitted as a courtesy to our patients. Any treatment not pre-authorized will be the financial responsibility of the patient at the time of service. Average time for insurance companies to process pre-authorizations is 4 to 6 weeks. Any insurance reimbursement will be refunded to the patient once it is received by our office. For treatment that is pre-authorized, only the amount not covered by insurance, will be due at the time of treatment. Any objections or disputes regarding insurance remittance are the sole responsibility of the patient.

(Please note: Delta Dental will send insurance reimbursements directly to your home, therefore payments must be paid in full on the day services are rendered.)
PRE-AUTHORIZATIONS MUST BE FORMALLY REQUESTED BY THE PATIENT
